High-Risk Maternity Services & Comprehensive Prenatal Care

High Risk Maternity Care & Consultation Service

Women with a history of pregnancy risks or current medical complications receive ongoing care and management through this program. Patients are cared for by a team of Yale obstetrical specialists with active participation by medical subspecialists. Collaboration and communication between your practice and the Yale team optimizes care for your patients.

Long Wharf and Yale New Haven Hospital Based Practices

This team concept is also practiced in the our clinical practices which is designed for ongoing prenatal management of complex pregnancies. We follow our patients closely form the point of referral to delivery and beyond. With the primary nursing model, patients receive individualized attention, and continuity of care is guaranteed throughout the pregnancy. With a single phone call, our patients can reach a staff member that knows them well.

Yale offers convenient and comprehensive services to assist you and your patients with the ongoing management of prepregnancy and perinatal diabetes. Individualized glucose management, home glucose meter loans, one-on-one counseling and support are available to your patients by the Diabetic Primary Nurse and our Perinatologists.
